Client Intake Form and Agreement

Please take your time and read carefully. If you book prior to submitting this form, we will send you a copy to be completed at least 24 hours before your appointment. Honesty Policy

You will be required to complete a medical intake form and sign a liability release. Please understand that Massage Therapy is not safe for everyone, and in order to determine proper treatment, we need to know what’s going on. If you withhold important information, you could be putting yourself or your therapist at risk.

Cancellation Policy

We require at least 24 hour notice for any cancellations. Of course, emergencies can happen, and we ask that you give us as much time as possible to adjust our schedules.

If we arrive and you are unavailable or unable to receive massage because of medical conditions withheld or inebriation, this is considered a late cancel. You will be charged 50% of each appointment missed.

No Tolerance Policy

Every one of our Therapists is Licensed through the Virginia Board of Nursing, has proper business licenses and insurance.

We are professionals and will be treated as such. Any inappropriate behavior will not be tolerated. If you make any kind of sexual advance, your session will be terminated immediately and you will be charged full price. If at any time your therapist feels threatened, they reserve the right to defend themselves and authorities will be notified.
